Alpha used a razor wire to behead Helen. She also held it up proudly in front of her followers to prove her superiority. Alpha has already proved her brutality.

Angela Kang, the showrunner told Comicbook.com that she knew they needed something big to kick start the Whisperer arc. “It had to be something that would have affected Hilltop. And also a crucial character.”
Momentous deaths lead to the series progression and builds up to the climax of the ending. Fans can expect a storyline where Alpha would kill Rosita and Ezekiel. She would put up their heads in the similar way she did for Helen. Any twist in the story can be expected. And there is a lot of foreshadow on some major murders by the power-hungry Alpha.
